摘要:
目的:观察关节镜下带线锚钉内固定治疗胫骨髁间棘骨折的临床疗效和安全性。方法:2013年5月至2015年10月,采用关节镜下带线锚钉内固定治疗新鲜闭合性胫骨髁间棘骨折患者27例,男16例、女11例。年龄23～54岁,中位数37岁。左侧15例,右侧12例。按胫骨髁间棘骨折的Meyers-McKeever分型,Ⅱ型11例、Ⅲ型16例。合并半月板损伤7例,内侧副韧带Ⅰ度损伤5例。伤后至手术时间3～7 d,中位数4 d。术后随访观察切口、骨折愈合情况及并发症发生情况,并于术后6个月采用膝关节Lysholm功能评分标准评价疗效。结果:所有患者均获随访,随访时间9～20个月,中位数13个月。切口均甲级愈合; 骨折均获骨性愈合,愈合时间11～15周,中位数14周。抽屉试验及Lachman试验阴性。均无膝关节不稳等并发症发生。术后6个月采用膝关节Lysholm功能评分标准评价疗效,优17例、良9例、可1例。结论:采用关节镜下带线锚钉内固定治疗胫骨髁间棘骨折,创伤小,固定牢靠,骨折愈合率高,能促进膝关节功能的恢复,并发症少,值得临床推广应用。
