摘要:
目的:观察关节镜下可吸收螺钉内固定治疗儿童胫骨髁间嵴骨折的临床疗效及安全性。方法:2005年1月至2014年6月,采用关节镜下可吸收螺钉内固定治疗儿童胫骨髁间嵴骨折患者7例,男5例、女2例。年龄6~13岁,中位数11岁。左侧4例,右侧3例。合并内侧半月板后角损伤1例、内侧副韧带损伤1例。按照胫骨髁间嵴骨折的Meyers-McKeever-Zaricnyi分类,Ⅱ型2例、Ⅲ型5例。受伤至手术时间2~7 d,中位数5 d。术后随访观察患者骨折愈合、关节功能恢复及并发症发生情况。结果:手术时间45~60 min,中位数49 min。所有患者均获随访,随访时间6~16个月,中位数8个月。骨折均愈合,愈合时间3~5个月,中位数4个月。均未出现感染、骨折端移位、股骨髁间窝撞击综合征、创伤性关节炎等并发症。Lachman试验、前抽屉试验及轴移试验均阴性,膝关节活动功能良好。治疗前Lysholm膝关节功能评分为(7.4±3.5)分、治疗后为(94.5±3.2)分。结论:关节镜下可吸收螺钉内固定治疗儿童胫骨髁间嵴骨折,具有固定可靠、骨折愈合好、并发症少等优点,有助于促进膝关节功能恢复。
