摘要:
目的:观察Dynesys动态固定系统治疗单节段腰椎间盘突出症的临床疗效。方法:2011年3月至2012年2月,采用Dynesys动态固定系统治疗单节段腰椎间盘突出症患者28例,男16例,女12例。年龄42~72岁,中位数60岁。L4~5椎间盘突出21例,L5S1椎间盘突出7例。术后随访观察并发症发生、腰椎疼痛缓解及功能恢复情况。结果:手术切口9~14 cm,中位数12 cm。手术时间95~135 min,中位数110 min。术中出血量200~400 mL,中位数330 mL。1例出现硬脊膜破裂,术中未行修补。所有患者均获得随访,随访时间24~33个月,中位数28个月。2例出现螺钉轻微松动,无明显临床症状,均未行特殊治疗。均无神经损伤、切口感染、内固定断裂等并发症发生。术后6、12、24个月,腰背部疼痛视觉模拟评分由术前(5.7±2.6)分降至(2.2±0.6)分、(2.2±0.7)分、(2.1±0.6)分,下肢疼痛视觉模拟评分由术前(5.5±1.3)分降至(0.7±0.5)分、(0.5±0.3)分、(0.4±0.3)分,Oswestry功能障碍指数由术前(57.0±18.0)%降至(17.0±5.0)%、(18.0±6.0)%、(15.0±5.0)%,无疼痛步行距离由术前400 m均升至2000 m以上。结论:采用Dynesys动态固定系统治疗单节段腰椎间盘突出症,能明显缓解腰腿疼痛症状,有利于腰椎功能的恢复,创伤小,并发症少,值得临床推广应用。
