摘要:
目的:探讨建立兔股骨骨缺损并骨延长模型的有效方法。方法:将36只3~4月龄健康新西兰白兔随机分为实验组和对照组。将自制骨延长牵张器安装于2组动物左侧股骨上后,于股骨中段截骨,做长约12 mm的骨缺损,其中实验组截骨时保护股骨外侧骨膜的连续性,对照组行全层截骨而未保护骨膜。静息期1周后开始进行骨延长。于骨延长结束后第28天,摄股骨X线片,观察骨痂生长情况,并取股骨标本进行肉眼大体观察。结果:①股骨标本X线检查结果。实验组股骨干延长区域呈高密度、均匀影像,新生骨质区与两断端边界模糊,外层皮质连续,可见大量新生骨痂生长; 对照组股骨干延长区域呈较高密度、不均匀影像,新生骨质区与股骨近端骨干连接部分边界模糊,与股骨远端骨干连接部分边界较清,尚存台阶,外层皮质尚未完全连续,可见新生骨痂生长。②股骨标本肉眼观察结果。实验组骨延长区域骨质连续,连接完整,骨痂量多,新生骨两端界线模糊; 对照组骨延长区域骨质相对连续,连接相对完整,见骨痂生长,新生骨远端尚有部分缺损,未完全填充骨缺损区。结论:在股骨截骨过程中保留部分骨膜,可以更有效地建立兔股骨骨缺损并骨延长模型。
Abstract:
ABSTRACT Objective:To explore the effective method of building rabbit model of femoral defect and lengthening.Methods:Thirty-six 3-4-month-old healthy New Zealand white rabbits were randomly divided into experimental group and control group.The self-made bone lengthening-stretching device was fixed in the left femur of the rabbit,and osteotomy of middle femur was performed to make 12 mm long bone defect.The continuity of femoral lateral periosteum was protected when osteotomy was performed in experimental group,while the periosteum was not protected and full-thickness osteotomy was performed in control group.The bone lengthening surgery was carried out after one-week rest period.At the 28th day after the end of bone lengthening,the femoral X-ray films were taken,and the bony callus growth was observed,and then macroscopic observation was performed on the specimens of femurs.Results:The X-ray examination results of femur specimens demonstrated that in experimental group the bone lengthening area of femoral shaft presented with high density and homogeneous imaging,and the boundary between newly formed bone region and the two broken ends was fuzzy,meanwhile,the continuous outer cortex and a large number of newly formed bony calluses were found; while in control group the bone lengthening area of femoral shaft presented with high density and inhomogeneous imaging,and the boundary between newly formed bone region and the proximal femoral shaft was fuzzy,and the boundary between newly formed bone region and the distal femoral shaft was clear,meanwhile,the discontinuous outer cortex and some newly formed bony calluses were found.The results of macroscopic observation on femur specimens demonstrated that continuous bone tissues and a large number of bony calluses were found in the bone lengthening area of femoral shaft in experimental group,and the two ends of the newly formed bone were fuzzy; while partly continuous bone tissues and some bony calluses were found in the bone lengthening area of femoral shaft in control group,and partial bone defects could be found in the distal end of newly formed bone.Conclusion:The femoral defect and lengthening rabbit model can be built more effectively by preserving some periosteum in the process of the femoral osteotomy.
