摘要:
目的:探讨亚洲型股骨近端防旋髓内钉(proximal femoral nail antirotation-Ⅱ,PFNA-Ⅱ)内固定治疗老年股骨转子间骨折的临床疗效及安全性。方法:2012年1月至2014年12月,采用PFNA-Ⅱ内固定治疗老年股骨转子间骨折患者65例,男27例、女38例。年龄60～93岁,中位数75岁。左侧34例,右侧31例。合并高血压病32例、糖尿病12例、冠心病5例、慢性阻塞性肺病2例。骨折按照Evans-Jensen分型,Ⅰa型14例、Ⅱa型16例、Ⅱb型23例、Ⅲ型12例。受伤至手术时间3～12 d,中位数6 d。术后随访观察骨折愈合、并发症发生及髋关节功能恢复情况。结果:手术时间27～75 min,中位数45 min。术中出血量50～150 mL,中位数80 mL。所有患者均获随访,随访时间10～24个月,中位数18个月。骨折均愈合,愈合时间10～14周,中位数12周。均未出现切口感染及螺旋刀片切割股骨头。3例出现螺旋刀片尾部退出,患肢短缩畸形; 1例出现髋内翻畸形。采用Harris髋关节功能评分标准评定疗效,本组优31例、良23例、可8例、差3例。结论:PFNA-Ⅱ内固定治疗老年股骨转子间骨折,手术时间短、出血量少、并发症少、髋关节功能恢复良好,值得临床推广应用。
