摘要:
目的:探讨老年股骨转子间骨折合并2型糖尿病的围手术期护理方法。方法:2012年1月至2013年12月,规范护理接受股骨近端防旋髓内钉内固定术治疗的老年股骨转子间骨折合并2型糖尿病患者57例,男23例、女34例。年龄61～89岁,中位数78岁。入院时空腹血糖7.3～21.6 mmol·L-1。合并高血压23例,冠心病7例,肺部疾病9例。术前护理包括血糖监测、健康教育、术前准备,术后护理包括生命体征监测、用药监控、功能锻炼指导、糖尿病护理、并发症护理和出院指导。采用世界健康基金会中国糖尿病教育项目组提供的糖尿病知识问卷评估患者对宣教知识的掌握情况,并观察血糖控制、切口愈合、并发症发生和患肢功能恢复情况。结果:住院期间血糖6.2～13.8 mmol·L-1。切口均甲级愈合。2例出现压疮,经对症处理后痊愈; 1例出现左下肢深静脉血栓,经治疗后血栓得到控制。均无切口感染、肺炎、泌尿系感染等并发症发生。住院期间所有患者均未发生跌倒及坠床事件。对宣教知识掌握良好48例、一般6例、差3例。术后12个月,按照Harris髋关节功能评分标准评价疗效,优45例、良7例、可5例。结论:对老年股骨转子间骨折合并2型糖尿病患者进行系统规范的围手术期护理,不仅有利于手术的顺利完成,还可以促进患肢功能的恢复,减少术后并发症的发生。
