摘要:
目的:观察钉棒系统复位内固定联合经椎弓根撬拨植骨术治疗胸腰椎压缩性骨折的临床疗效和安全性。方法:2010年9月至2013年6月,采用钉棒系统复位内固定联合经椎弓根撬拨植骨术治疗胸腰椎压缩性骨折患者57例,男36例、女21例。年龄21~62岁,中位数37岁。损伤椎体,T114例、T1223例、L125例、L25例。脊髓损害按美国脊髓损伤协会分级,B级1例、C级9例、D级13例、E级34例。术后随访观察并发症发生、伤椎高度恢复、后凸畸形纠正、植骨愈合和脊髓神经恢复情况。结果:所有患者均获随访,随访时间6~24个月,中位数18个月。切口均甲级愈合。均未出现断钉、断棒、螺钉松动等并发症。术后4周与末次随访时Cobb角分别由术前22.75°± 6.58°降至5.21°±2.21°、5.33°±2.32°。术后4周与末次随访时伤椎前缘压缩率分别由术前(51.45±16.12)%降至(5.25±2.26)%、(5.95±3.48)%; 伤椎后缘压缩率分别由术前(27.78±11.44)%降至(4.14±1.65)%、(4.43±2.03)%。末次随访时,CT扫描显示植骨愈合良好; 按美国脊髓损伤协会分级,B级1例、C级2例、D级8例、E级46例。结论:采用钉棒系统复位内固定联合经椎弓根撬拨植骨术治疗胸腰椎压缩性骨折,能有效恢复椎体高度,纠正后凸畸形,促进脊髓神经恢复,且并发症少,值得临床推广应用。
