摘要:
目的:探讨锁定钢板内固定联合自体髂骨植骨治疗股骨近端粉碎性骨折的临床疗效和安全性。方法:2012年1月至2014 年1月收治32例新鲜股骨近端粉碎性骨折患者,男24例,女8例; 年龄20～64岁,中位数40.5岁; 左侧14例,右侧18例; 车祸伤17例,高处坠落伤8例,摔伤7例。转子间骨折19例,其中EvansⅢ型8例、Ⅳ型7例、Ⅴ型4例; 转子下骨折13例,其中Russell-TayloⅠa型2例、Ⅰb型3例、Ⅱa型4例、Ⅱb型4例。5例患者合并其他部位骨折。采用锁定钢板内固定联合自体髂骨植骨治疗,治疗结束后评定临床疗效和安全性。结果:术后所有骨折均达到解剖复位或接近解剖复位。所有患者均获随访,随访时间3～9个月,中位数6个月。骨折均在12～24周达到骨性愈合标准。至随访结束时未发生感染、髋内翻畸形、内固定松动、断裂等并发症。按照Sanders髋关节创伤后功能评分标准评定疗效,优26例、良6例。结论:采用锁定钢板内固定联合自体髂骨植骨治疗股骨近端粉碎性骨折,安全性高、固定可靠、骨折愈合快、关节功能恢复好,是治疗该类骨折的有效方法。
