摘要:
目的:探讨CT改良骨窗在急性隐匿性骨质疏松性椎体压缩骨折(osteoporotic vertebral compression fracture,OVCF)诊断中的应用价值。方法:纳入46例急性OVCF患者的胸腰椎CT图像,由2名影像专业副主任医师在PACS工作站分别采用常规骨窗(窗宽1500 Hu、窗位500 Hu)和改良骨窗(窗宽400 Hu、窗位214 Hu)诊断急性隐匿性OVCF,记录诊断结果及阅片时间,评价2名医师诊断结果的一致性; 与MRI诊断结果比较,分析常规骨窗和改良骨窗诊断急性隐匿性OVCF的敏感度、特异度、阳性预测值、阴性预测值和准确度。结果:共纳入367个椎体的CT图像,常规骨窗诊断急性隐匿性OVCF椎体38个,其中假阳性椎体4个; 改良骨窗诊断急性隐匿性OVCF椎体47个,其中假阳性椎体1个; MRI确诊急性OVCF椎体50个。2名医师诊断结果的一致性较好(Kappa值=0.856)。常规骨窗诊断急性隐匿性OVCF的敏感度为68.0%、特异度为98.7%、阳性预测值为89.5%、阴性预测值为95.1%、准确度为94.5%; 改良骨窗诊断急性隐匿性OVCF的敏感度为92.0%、特异度为99.7%、阳性预测值为97.9%、阴性预测值为98.7%、准确度为98.6%。改良骨窗诊断急性隐匿性OVCF的敏感度、阴性预测值、准确度均高于常规骨窗(χ2=9.000,P=0.003; χ2=7.091,P=0.008; χ2=12.960,P=0.000)、阅片时间短于常规骨窗[(58.1±12.4)s,(117.5±27.1)s,t=39.873,P=0.000],2种骨窗诊断急性隐匿性OVCF的特异度、阳性预测值比较,组间差异无统计学意义(χ2=1.814,P=0.178; χ2=2.593,P=0.107)。结论:采用CT改良骨窗诊断急性隐匿性OVCF,与常规骨窗相比,敏感度、阴性预测值和准确度高,且阅片时间短,具有较高的应用价值。
