摘要:
目的:观察微创经皮钢板内固定术(minimally invasive percutaneous plate osteosynthesis,MIPPO)治疗老年肱骨干长斜形骨折的临床疗效及安全性。方法:2016年1月至2019年6月,采用MIPPO治疗肱骨干骨折患者42例。男26例,女16例。年龄 65～85岁,中位数72岁。均为闭合性长斜形骨折; 左侧28例,右侧14例; 中上段骨折30例,中下段骨折12例。根据AO骨折分型标准,A型9例、B型26例、C型7例。受伤至手术时间1～7 d,中位数3.5 d。随访观察骨折愈合及并发症发生情况,测定肩、肘关节活动度及握力。末次随访时,分别采用美国加州大学洛杉矶分校(the University of California at Los Angeles,UCLA)肩关节评分标准和Mayo肘关节评分标准评定肩关节和肘关节功能。结果:所有患者均获随访,随访时间12～24个月,中位数17.5个月。骨折均骨性愈合,愈合时间3～5个月,中位数3.5个月。术后1个月,肩关节前屈活动度150.0°±7.5°、外展活动度135.0°±5.3°、外展外旋活动度50.5°±2.2°、外展内旋活动度59.5°±4.2°,肘关节屈曲活动度105.0°±3.5°; 末次随访时,肩关节前屈活动度172.5°±5.5°、外展活动度169.0°±4.2°、外展外旋活动度62.5°±3.7°、外展内旋活动度70.5°±2.1°,肘关节屈曲活动度136.5°±7.2°。末次随访时,患侧握力(223±15)N、健侧握力(231±11)N,肘关节内外翻畸形3.2°±0.7°、前后成角3.4°±0.5°; UCLA肩关节评分(32.1±1.5)分,优35例、良7例; Mayo肘关节评分(93.5±1.7)分,均为优。2例术中复位失败,行骨折端有限切开辅助复位。均未发生感染、内固定失效、骨折移位、骨不连、桡神经损伤等并发症。结论:采用MIPPO治疗老年肱骨干长斜形骨折,骨折愈合好,有利于肩、肘关节功能恢复,且并发症少。
