摘要:
目的:观察手法复位经皮克氏针内固定治疗儿童闭合性肱骨远端全骨骺分离的临床疗效和安全性。方法:2012年1月至2014年6月,采用手法复位经皮克氏针内固定治疗儿童闭合性肱骨远端全骨骺分离患者22例,男14例、女8例。年龄1.5～6.5岁,中位数3.5岁。左侧12例,右侧10例。按骨骺损伤的Salter-Harris分型,Ⅰ型13例、Ⅱ型9例。合并桡神经损伤1例。受伤至就诊时间1 h至6 d,中位数2 d。术后随访观察骨折愈合、并发症发生及患肢功能恢复情况。结果:所有患者均获随访,随访时间6～24个月,中位数14个月。骨折均愈合。1例发生尺神经损伤,拔除尺侧克氏针并给予营养神经药物治疗后3个月症状消失; 所有患者均未发生前臂缺血性肌挛缩、针道感染、肘内外翻畸形、骨化性肌炎等并发症。术后6个月,采用Flynn肘关节功能评分标准评价疗效,优12例、良7例、可3例。结论:采用手法复位经皮克氏针内固定治疗儿童闭合性肱骨远端全骨骺分离,创伤小,骨折愈合率高,有利于肘关节功能的恢复,并发症少,值得临床推广应用。
