摘要:
目的:探讨电针联合玻璃酸钠关节腔注射治疗踝关节创伤性关节炎(traumatic arthritis,TA)的临床疗效。方法:将符合要求的96例踝关节TA患者随机分为2组,每组48例。联合治疗组采用电针联合玻璃酸钠关节腔注射治疗,关节注射组采用玻璃酸钠关节腔注射治疗。电针治疗每天1 次,5 d为1个疗程,间隔2 d后继续下一疗程,共治疗4个疗程; 玻璃酸钠关节腔注射治疗每周1次,共治疗4次。分别于治疗前后采用Maryland足部评分标准从疼痛和功能2个方面进行疗效评定。结果:至治疗结束时,2组均无患者脱落。治疗前2组患者的Maryland评分比较,差异无统计学意义[(67.76±9.17)分,(65.13±8.61)分,t=1.502,P=0.140]; 治疗后2组患者的评分均增加(t=2.862,P=0.016; t=14.605,P=0.653),联合治疗组的评分高于关节注射组[(85.57±5.29)分,(78.96±8.49)分,t=5.257,P=0.000]。结论:电针联合玻璃酸钠关节腔注射治疗能有效减轻踝关节TA患者的疼痛症状,改善踝关节功能,疗效优于单纯玻璃酸钠关节腔注射治疗。
Abstract:
ABSTRACT Objective:To explore the clinical curative effect of electroacupuncture therapy combined with intra-articular injection of sodium hyaluronate in the treatment of traumatic arthritis(TA)of ankle joint.Methods:Ninety-six patients with ankle TA were enrolled in the study and randomly divided into 2 groups,48 cases in each group.The patients were treated with combination therapy of electroacupuncture and intra-articular injection of sodium hyaluronate(group A)and monotherapy of intra-articular injection of sodium hyaluronate(group B)respectively.The electroacupuncture therapy were performed on the patients once a day for consecutive 4 course of treatment,5 days for each course with a 2-day rest-insertion between courses.The intra-articular injection of sodium hyaluronate were performed on the patients once a week for 4 times.The curative effects on ankle pain and ankle function were evaluated by using Maryland foot scores before and after treatment respectively.Results:No patients dropped out of the 2 groups at the end of the treatment.There was no statistical difference in Maryland scores between the 2 groups before the treatment(67.76+/-9.17 vs 65.13+/-8.61 points,t=1.502,P=0.140).The Maryland scores increased in both of the 2 groups after the treatment(t=2.862,P=0.016; t=14.605,P=0.653),and the Maryland scores were higher in group A compared to group B(85.57+/-5.29 vs 78.96+/-8.49 points,t=5.257,P=0.000).Conclusion:The combination therapy of electroacupuncture and intra-articular injection of sodium hyaluronate can effectively relieve the ankle pain and improve the ankle function in patients with ankle TA,and its clinical curative effects is better than that of monotherapy of intra-articular injection of sodium hyaluronate.
