摘要:
目的:观察撬拨复位双向空心螺钉内固定联合植骨治疗SandersⅡ、Ⅲ型跟骨骨折的临床疗效和安全性。方法:2011年1月至2015年6月,采用撬拨复位双向空心螺钉内固定联合植骨治疗SandersⅡ、Ⅲ型跟骨骨折患者28例33足,男21例25足、女7例8足。年龄23～56岁,中位数35岁。左足10例、右足13例,双足5例。按照跟骨骨折Sanders分型,Ⅱ型18例22足、Ⅲ型10例11足。受伤至手术时间2～13 d,中位数6 d。术后随访观察跟骨宽度、Böhler角、Gissane角的恢复情况,以及骨折愈合和并发症发生情况,并采用Maryland足部评分标准评价疗效。结果:手术时间30～60 min,中位数41 min。术中出血量10～30 mL,中位数17 mL。所有患者均获随访,随访时间12～28个月,中位数17个月。骨折均愈合。术后3个月,跟骨宽度由术前(33.82±2.76)mm减少至(30.43±2.04)mm,Böhler角由术前13.26°±4.35°增大至32.41°±4.14°,Gissane角由术前128.55°±9.71°减小至120.84°±9.26°,Maryland足部评分由术前(54.17±3.09)分增加至(85.24±5.67)分。均未出现皮缘坏死、感染等并发症。术后12个月,Maryland足部评分(86.41±4.88)分,优16例19足、良10例12足、可2例2足。结论:采用撬拨复位双向空心螺钉内固定联合植骨治疗SandersⅡ、Ⅲ型跟骨骨折,创伤小,能促进骨折愈合,恢复跟骨基本形态,有利于足部功能的恢复,且并发症少,值得临床推广应用。
