摘要:
目的:观察跗骨窦切口微创接骨板内固定治疗SandersⅡ、Ⅲ型跟骨骨折的临床疗效及安全性。方法:2013年12月至2015年12月,采用跗骨窦切口微创接骨板内固定治疗SandersⅡ、Ⅲ型跟骨骨折患者32例,男20例、女12例。年龄16~58岁,中位数45岁。均为新鲜闭合性骨折,其中左侧14例、右侧16例、双侧2例。跟骨骨折类型,SandersⅡ型13例、Ⅲ型19例。受伤至手术时间6~10 d,中位数8 d。随访观察患足功能恢复及并发症发生情况,并比较手术前后的Böhler角、Gissane角、跟骨高度和跟骨宽度。结果:所有患者均获随访,随访时间3~12个月,中位数7个月。所有患者骨折均愈合,愈合时间3~4个月,中位数3.5个月。均未出现切口感染、内固定物外露及创伤性关节炎,2例患者出现跟骨疼痛。末次随访时的Böhler角、Gissane角、跟骨高度及跟骨宽度均较术前改善[(1.3°±0.5°),(32.3°±4.7°),t=-5.783,P=0.000;(141.6°±8.9°),(123.2°±7.8°),t=3.013,P=0.000;(29.1±3.9)mm,(38.6±4.8)mm,t=-3.753,P=0.000;(34.6±3.1)mm,(26.7±3.6)mm,t=4.352,P=0.000]。采用美国足与踝关节协会踝与后足功能评分标准评定疗效,优16例、良14例、可2例。结论:跗骨窦切口微创接骨板内固定治疗SandersⅡ、Ⅲ型跟骨骨折,骨折愈合率高、跟骨形态和足踝功能恢复好、并发症少,值得在基层医院推广应用。
