摘要:
目的:探讨掌侧万向锁定加压钢板内固定治疗桡骨远端分水岭线以远骨折的临床疗效及安全性。方法:2014年6月至2015年6月,采用掌侧万向锁定加压钢板内固定治疗桡骨远端分水岭线以远骨折患者12例,男4例、女8例。年龄42～72岁,中位数56.5岁。均为闭合性损伤,左侧7例、右侧5例。均有腕关节畸形、疼痛及活动受限等临床表现。8例合并月骨窝关节面塌陷。均未合并血管神经损伤。根据骨折AO分型,C1型8例、C3型4例。4例为伤后2周内首次接受手术治疗,8例为手法复位失败2个月后接受手术治疗。术后随访观察骨折愈合、并发症发生及腕关节功能恢复等情况。结果:手术时间60～90 min,中位数75 min。所有患者均获随访,随访时间8～13个月,中位数10个月。骨折均愈合,愈合时间8～12周,中位数10周。均未出现皮肤坏死、肌腱断裂、前臂骨筋膜室综合征、腕管综合征及内固定失败等并发症。治疗前桡骨茎突高度(6±1)mm,桡骨掌倾角20°±3°、尺偏角28°±2°; 末次随访时桡骨茎突高度(12±1)mm,桡骨掌倾角10°±3°、尺偏角21°±3°。腕关节活动度,掌屈71°±4°、背伸66°±3°、尺偏22°±3°、桡偏17°±1°。采用中华医学会手外科学分会上肢部分功能评定试用标准中腕关节功能评定标准评价疗效,优8例、良2例、可2例。结论:掌侧万向锁定加压钢板内固定治疗桡骨远端分水岭线以远骨折,具有手术时间短、骨折愈合率高、并发症少、腕关节功能恢复良好等优点,值得临床推广应用。
