摘要:
目的:观察球囊扩张椎体后凸成形术治疗骨质疏松性椎体压缩骨折的疗效和安全性。方法:应用球囊扩张椎体后凸成形术 治疗椎体压缩性骨折患者128 例152椎,男36例,女92例; 年龄58~89岁,中位数73.5岁。椎体受累节段:T63 节,T74节,T84节,T95节,T106节,T1118节,T1237 节,L141节,L223节,L36节,L45节。术后观察疼痛缓解及椎体高度恢复情况。 结果:128例患者,均行脊椎双侧骨水泥灌注,单节椎体骨水泥灌注量为2.8~6.3 mL,中位数4.3 mL; 均获随访,随访时间7~13月, 中位数9.5个月。术后腰背部疼痛消失95例; 腰背部持续钝痛,3~6 d后消失28例; 疼痛无明显缓解5例。疼痛视觉模拟评分术前 7~10分,中位数8.5分; 术后0~4分,中位数2.5分。伤椎前缘高度术前13~17 mm,中位数15.5 mm; 术后21~25 mm,中位数23.5 mm; 后凸角术前28°~30°,中位数29.5°; 术后14°~17°,中位数15.5°。均无感染及其他组织损伤发生; 骨水泥侧漏8例, 但未出现明显临床症状。结论:球囊扩张椎体后凸成形术治疗骨质疏松性椎体压缩骨折具有可有效缓解疼痛、恢复椎体高度、术 后并发症少的优点,可作为治疗该病的首选方法。
