摘要:
目的:观察踝关节镜下韧带解剖重建术治疗距腓前韧带距骨侧断裂的临床疗效和安全性。方法:2019年1月至2023年1月,采用踝关节镜下韧带解剖重建术治疗距腓前韧带距骨侧断裂患者15例。男10例,女5例。年龄18～30岁,中位数25岁。均为新鲜闭合性损伤。左侧7例,右侧8例。8例合并距骨止点处撕脱骨折。分别于术前和术后3个月,采用疼痛视觉模拟量表(visual analogue scale,VAS)评价踝关节疼痛情况,采用美国足与踝关节协会(American Orthopaedic Foot and Ankle Society,AOFAS)踝与后足评分标准评价踝关节功能; 于术前和末次随访时,采用Tegner运动水平评级标准评价患者运动能力。随访观察并发症发生情况。结果:本组15例,手术均顺利完成。所有患者均获随访,随访时间6～16个月,中位数12个月。踝关节疼痛VAS评分,术前(6.27±1.53)分,术后3个月(0.73±0.70)分。AOFAS踝与后足评分,术前(34.00±9.93)分,术后3个月(93.67±3.29)分。Tegner运动水平评级,术前1～3级,中位数2级; 末次随访时5～6级,中位数6级。均未发生切口感染、皮肤坏死、韧带再断裂等并发症。结论:踝关节镜下韧带解剖重建术治疗距腓前韧带距骨侧断裂,能够缓解踝关节疼痛,改善踝关节功能与运动能力,且安全性高。
