摘要:
脊髓型颈椎病(cervical spondylotic myelopathy,CSM)一旦确诊,经规范非手术治疗无效或神经功能障碍进行性加重,应尽早手术治疗,但在具体术式选择方面尚存争议。传统前路颈椎间盘切除融合术(anterior cervical discetomy and fusion,ACDF)是治疗CSM的金标准; 但融合术后手术节段运动功能丧失,致其相邻节段应力增加,存在加速相邻节段退变(adjacent segment degeneration,ASD)的可能。颈椎人工椎间盘置换术(artificial cervical disc replacement,ACDR)可维持病变节段活动度,理论上可减少ASD的发生。对于CSM前路手术中的融合与非融合问题,有必要进行深入探讨,以利于手术方式的合理选择。本文分别对ACDF、颈椎前路椎体次全切除融合术、ACDR和前路“杂交式”减压术的优缺点进行了总结和评述。
