微型钛板内固定治疗Lisfranc损伤()

摘要:
目的:观察微型钛板内固定治疗Lisfranc损伤的临床疗效和安全性。方法:2014年6月至2015年12月,采用微型钛板内固定治疗Lisfranc损伤患者16例17足,男12例13足、女4例4足。年龄17～68岁,中位数43岁。闭合性损伤14例15足,开放性损伤2例2足。合并跖骨近端骨折17足、楔骨骨折15足、舟骨骨折7足、骰骨骨折4足、Chopart关节损伤3足。受伤至手术时间1～9 d,中位数6 d。术后随访观察骨折复位、愈合及并发症发生情况,并于末次随访时采用美国足与踝关节协会中部足功能评分标准评价疗效。结果:所有患者均获随访,随访时间6～25个月,中位数13个月。骨折均获解剖复位及坚强固定。术后3个月骨折均一期愈合。1例1足发生钛板断裂; 2例2足出现跖跗关节创伤性关节炎; 1例1足术后切口窦道形成,经定期换药2个月后窦道消失、切口愈合; 其余患者均无钛板断裂、创伤性关节炎、切口感染、皮肤坏死等并发症发生。末次随访时,按照美国足与踝关节协会中足功能评分标准评价疗效,优7例7足、良8例9足、可1例1足。结论:采用微型钛板内固定治疗Lisfranc损伤,固定牢靠,可获得良好的解剖复位,骨折愈合率高,有利于患足功能的恢复,并发症少,值得临床推广应用。
