摘要:
目的:观察切开复位螺钉内固定治疗Lisfranc损伤的临床疗效和安全性。方法:2010年3月至2014年6月,采用切开复位螺钉内固定手术治疗Lisfranc损伤患者21例,男12例、女9例; 年龄19～54岁,中位数34岁; 左侧10例,右侧11例。Myerson分型,A型6例、B1型4例、B2型7例、B3型1例、C型3例。术后随访观察骨折复位、愈合情况及并发症发生情况,并在末次随访时,分别采用疼痛视觉模拟量表(visual analogue scale,VAS)和美国足踝外科协会(American orthopaedic foot and ankle society,AOFAS)踝与后足功能评分标准评估患足疼痛和功能情况。结果:本组21例患者均获随访,随访时间24～31个月,中位数26个月; 骨折均达解剖复位,愈合良好,愈合时间8～12周,中位数10周。末次随访时,本组VAS评分(0.6±0.9)分,AOFAS评分(87.1±7.2)分。术后并发切口边缘皮肤坏死4例,经换药后切口愈合。术后3个月并发患足行走疼痛5例,经理疗、抗炎、镇痛及中药薰洗治疗3个月后疼痛缓解。均无内固定断裂、骨折再移位及创伤性关节炎等并发症发生。结论:切开复位螺钉内固定治疗Lisfranc损伤,骨折复位、愈合良好,有利于患足疼痛缓解和功能恢复,安全可靠。
