肩关节镜下垂直缝合联合滑轮技术治疗Ⅱ型肩关节上盂唇前后向损伤()

摘要:
目的:探讨肩关节镜下垂直缝合联合滑轮技术治疗Ⅱ型肩关节上盂唇前后向(superior labrum anterior and posterior,SLAP)损伤的临床疗效和安全性。方法:2018年2月至2023年6月,收治17例Ⅱ型SLAP损伤患者。男11例,女6例; 年龄48～65岁,中位数55岁; 跌伤8例,投掷运动损伤2例,羽毛球运动损伤4例,乒乓球运动损伤3例; 合并肩袖轻度撕裂11例,合并冈上肌腱炎2例; 病程6～14个月,中位数10个月。均采用肩关节镜下垂直缝合联合滑轮技术治疗。采用Constant-Murley评分标准、疼痛视觉模拟量表(visual analogue scale,VAS)、美国肩肘外科协会(American shoulder and elbow surgeons,ASES)评分标准,评价肩关节功能及疼痛程度; 术后12个月随访时,根据MRI检查结果评估修复的上盂唇及肱二头肌长头肌腱愈合情况。同时记录患者重返运动情况及并发症发生情况。结果:所有患者均获随访,随访时间12～24个月,中位数18个月。术后12个月随访时,本组患者的Constant-Murley评分由术前(60.65±4.26)分提升至(94.19±1.94)分、ASES评分由术前(6.59±1.94)分提升至(13.57±0.51)分、肩部疼痛VAS评分由术前(6.94±1.03)分降至(0.31±0.50)分; MRI显示修复的上盂唇及肱二头肌长头肌腱延续性尚可,未见再撕裂; 15例患者恢复伤前运动,2例因投掷疼痛放弃伤前运动,但不影响日常生活,故未做进一步治疗。所有患者均未出现感染、神经损伤、锚钉断裂和滑脱等并发症。结论:应用肩关节镜下垂直缝合联合滑轮技术治疗Ⅱ型SLAP损伤,可解剖重建上盂唇-肱二头肌长头肌腱复合体,有效缓解肩部疼痛症状、改善肩关节功能,而且具有较高的安全性。
