摘要:
目的:观察Ⅰ型牛胶原蛋白生物膜预防屈指肌腱损伤术后粘连的临床疗效和安全性。方法:按要求选取72例手部Ⅱ区新鲜屈指肌腱损伤患者,男43例,女29例。年龄16~55岁,中位数37.5岁。实验方案通过医院伦理委员会的论证。采用随机数字表将72例患者随机分为2组,每组36例。2组均采用改良Kessler肌腱缝合法修复肌腱,治疗组用Ⅰ型牛胶原蛋白生物膜包绕吻合的肌腱断端,对照组在损伤局部不使用任何药物及特殊处理。观察记录患者治疗期间及治疗后血尿常规、生化指标及心电图有无异常,及有无肌腱再断裂等不良事件发生。并于术后第8周采用中华医学会手外科学会手部肌腱修复后评定标准评定2组患者的手部肌腱总主动活动度。结果:①安全性。2组患者术后切口均一期愈合,无感染及肌腱再次断裂; 血尿常规、生化指标及心电图未见异常。②手部肌腱总主动活动度。治疗组优19例,良11例,可5例,差1例; 对照组优8例,良10例,可13例,差5例。治疗组手部肌腱总主动活动度优于对照组(Z=-3.229,P=0.001)。结论:Ⅰ型牛胶原蛋白生物膜能安全、有效地预防屈指肌腱损伤术后粘连。
Abstract:
Objective:To observe the clinical curative effect and safety of biomembrane made of bovine type-Ⅰcollagen protein in the prevention of postoperative finger flexor tendon adhesion formation.Methods:Seventy-two patients with fresh flexor tendon injuries in zoneⅡof hands were selected according to the requirements,male 43 cases,while female 29 cases,ranging in age from 16 to 55 years with a median of 37.5 years.Research Ethics Committee approval was obtained.The 72 patients were randomly divided into 2 groups according to random digits table,36 cases in each group.The injured tendons of patients in the 2 groups were all repaired through modified Kessler tendon suture.Cases in the treatment group were administrated with biomembrane made of bovine type-Ⅰcollagen protein surrounded anastomotic broken ends of tendon,while the others in the control group were administrated without any drug or special treatment in the local injuries.The results of routine blood test,routine urine test,biochemical indicator test and electrocardiogram were observed and recorded during and after the treatment,and the occurrence of tendon re-rupture and other adverse events were also observed and recorded.The total active range of motion of hand tendons in the 2 groups were evaluated according to the repaired hand tendon evaluation standards set up by hand surgery branch of Chinese Medical Association at the 8th week after the operation.Results:①Safety.All of the patients in the 2 groups got primary healing in the operative incisions,without infection and tendon re-rupture; and no abnormal results of routine blood test,routine urine test,biochemical indicator test and electrocardiogram were found in the 2 groups.②Total active range of motion of hand tendons.Nineteen patients got excellent outcomes,11 good,5 fair and 1 poor in the treatment group; while 8 patients got excellent outcomes,10 good,13 fair and 5 poor in the control group.The total active range of motion of hand tendon in the treatment group was better than that in the control group(Z=-3.229,P=0.001).Conclusion:Biomembrane made of bovine type-Ⅰcollagen protein can safely and effectively prevent postoperative finger flexor tendon adhesion formation.
