摘要:
目的:观察生物型加长柄人工股骨头置换术治疗高龄不稳定性股骨转子间骨折的临床疗效和安全性。方法:2009年8月至2015年1月,采用生物型加长柄人工股骨头置换术治疗高龄不稳定性股骨转子间骨折患者48例,男12例、女36例。年龄78~93岁,中位数79岁。左侧19例,右侧29例。按照股骨转子间骨折的AO分型,A2.2型39例、A2.3型9例。合并糖尿病5例,高血压20例。术后随访观察骨折愈合、并发症发生情况,并于末次随访时采用Harris髋关节评分标准评价疗效。结果:手术时间50~110 min,中位数61 min; 术中失血量200~550 mL,中位数290 mL。所有患者均获随访,随访时间12~15个月,中位数14个月。骨折均愈合。1例术后切口感染,经清创、静脉滴注抗生素后感染得到控制; 2例术后患肢短缩1.5 cm,因未出现症状而未做任何处理; 2例关节假体出现轻微下沉并伴有大腿疼痛,给予口服止痛药后疼痛得到控制。髋关节功能均无受限,均无假体周围骨折、髋关节脱位、假体无菌性松动、异位骨化等并发症发生。末次随访时,Harris评分(78.8±8.2)分,优32例、良11例、可5例。结论:采用生物型加长柄人工股骨头置换术治疗高龄不稳定性股骨转子间骨折,创伤小,手术时间短,骨折愈合率高,能促进患肢功能的恢复,并发症少,值得临床推广应用。
