摘要:
目的:探讨Ilizarov外固定支架结合克氏针固定治疗Pilon骨折的临床疗效及安全性。方法:2009年2月至2014年7月,采用Ilizarov外固定支架结合克氏针固定治疗Pilon骨折患者11例,男8例、女3例; 年龄13～76岁,中位数45岁; 左侧7例,右侧4例。闭合性骨折7例,开放性骨折4例(均为GustiloⅠ型)。胫腓骨远端骨折AO分型,B型4例、C型7例。术后随访观察骨折愈合、踝关节功能恢复及并发症发生情况。结果:本组11例患者均获随访,随访时间12～39个月,中位数18个月; 骨折均愈合,愈合时间12～24周,中位数12周。术后1周,根据X线及CT检查结果,依据Anglen标准对骨折复位情况进行评估,本组优7例、良4例。术后12个月,依据美国足与踝关节协会踝与后足功能评分标准评价患足功能,本组优8例、良3例。均未发生针道感染、皮肤坏死及切口感染等并发症。结论:Ilizarov外固定支架结合克氏针固定治疗Pilon骨折,可维持骨折复位、固定可靠,有利于骨折愈合和患肢功能恢复,并发症少,安全可靠。
