摘要:
目的:探讨氟脱氧葡萄糖F18(fluorodeoxyglucose F18,18F-FDG)正电子发射断层扫描及电脑断层扫描(positron emission tomography and computed tomography,PET/CT)在未知原发灶肿瘤骨转移(bone metastasis of unknown primary origin,BMUO)诊断中的价值。方法:选择行18F-FDG PET/CT检查的223例未确定原发灶的转移性骨肿瘤患者的病例资料进行分析。男122例,女101例。年龄40～89岁,中位数62岁。所有纳入研究的患者均在常规影像检查发现肿瘤骨转移病灶后3 d内行18F-FDG PET/CT检查,查找原发肿瘤,并通过穿刺、内镜检查、手术等方式取病变组织标本进行组织病理检查。18F-FDG PET/CT检查均采用Siemens Biograph Truepoint 64(52环)PET/CT。结果:在223例患者中,203例经18F-FDG PET/CT检查诊断为阳性,其中真阳性199例、假阳性4例; 20例诊断为阴性,其中真阴性14例、假阴性6例。18F-FDG PET/CT确定BMUO原发肿瘤的灵敏度为97.1%、特异度为77.8%、准确度为91.9%、阳性预测值为98.0%、阴性预测值为70.0%。假阳性共4例,其中结核2例、结节病1例、POEMS综合征 1例; 假阴性6例,其中胃印戒细胞癌1例、肾透明细胞癌1例、甲状腺癌1例、前列腺癌1例、膀胱癌2例。 18F-FDG PET/CT检查检测到了所有先前已知的转移灶,并在173例患者身上发现了新的转移灶。最终确诊的205例患者中,肺癌109例(53.2%)、结肠癌16例(7.8%)、食管癌13例(6.3%)、前列腺癌9例(4.4%)、乳腺癌9例(4.4%)、胃癌8例(3.9%)、卵巢癌7例(3.4%)、直肠癌7例(3.4%)、鼻咽癌6例(2.9%)、喉癌5例(2.4%)、肝癌4例(2.0%)、胰腺癌4例(2.0%)、肾癌3例(1.5%)、膀胱癌2例(1.0%)、多发骨髓瘤2例(1.0%)、甲状腺癌1例(0.5%)。按照肿瘤的组织病理学类型,腺癌共135例,约占65.8%(135/205)。结论:18F-FDG PET/CT是诊断BMUO原发肿瘤的一种可靠方法,可作为查找BMUO原发病灶的首选检查方法。
