功能锻炼对桡骨远端骨折拆除石膏外固定后 腕关节康复的影响()

摘要:
目的:探讨功能锻炼对桡骨远端骨折拆除石膏外固定后腕关节康复的影响。方法:2012年4月至2013年4月,指导40例拆除石膏外固定后的桡骨远端骨折患者进行功能锻炼,男19例、女21例。年龄18～65岁,中位数43岁。左侧24例,右侧16例。30例行手法复位,10例未行手法复位。石膏外固定时间4～7周,中位数6周。分别于功能锻炼前及功能锻炼后第1、第2、第4、第8周记录并比较患者腕关节疼痛视觉模拟评分、腕关节活动度、Mayo腕关节临床功能评分及握力情况。结果:腕关节疼痛视觉模拟评分分别由功能锻炼前(3.41±2.65)分降低至功能锻炼后第1周(2.41±1.88)分、第2周(2.03±1.54)分、第4周(1.36±1.53)分、第8周(0.72±0.94)分。握力减少百分数分别由功能锻炼前(74.6±21.56)%降低至功能锻炼后第1周(62.39±20.74)%、第2周(53.82±20.08)%、第4周(43.78±19.53)%、第8周(28.89±19.94)%。健、患侧腕关节背伸活动度差值由功能锻炼前48.74°±15.78°降至功能锻炼后第1周31.67°±16.75°、第2周23.83°±13.00°、第4周17.33°±14.07°、第8周9.77°±5.21°; 健、患侧腕关节掌屈活动度差值由功能锻炼前31.05°±11.31°降至功能锻炼后第1周20.72°±10.54°、第2周17.30°±7.68°、第4周14.20°±10.16°、第8周10.27°±9.23°; 健、患侧腕关节旋前活动度差值由功能锻炼前20.18°±9.34°降至功能锻炼后第1周13.05°±17.82°、第2周4.65°±9.75°、第4周2.75°±7.63°、第8周1.80°±4.49°; 健、患侧腕关节旋后活动度差值由功能锻炼前30.24°±22.15°降至功能锻炼后第1周21.00°±24.12°、第2周10.82°±15.63°、第4周4.31°±10.13°、第8周2.48°±6.14°; 健、患侧腕关节桡偏活动度差值由功能锻炼前13.67°±7.22°降至功能锻炼后第1周8.45°±6.11°、第2周4.15°±4.31°、第4周3.90°±5.12°、第8周2.65°±6.00°; 健、患侧腕关节尺偏活动度差值由功能锻炼前14.07°±10.21°降至功能锻炼后第1周11.25°±9.19°、第2周9.30°±7.78°、第4周5.80°±4.69°、第8周4.70°±4.24°。Mayo腕关节临床功能评分由功能锻炼前(47.31±13.37)分升至功能锻炼后第1周(53.97±13.37)分、第2周(64.23±10.91)分、第4周(70.38±11.20)分、第8周(82.18±08.25)分。结论:桡骨远端骨折拆除石膏外固定后早期行功能锻炼,能够缓解腕关节疼痛症状,改善腕关节活动度,提高握力,有利于腕关节功能的恢复。
