摘要:
目的:系统评价血清铁蛋白含量与骨密度的相关性。方法:应用计算机检索中国知网、万方数据库、PubMed、Embase、Web of Science和Cochrane Library中关于血清铁蛋白含量与骨密度相关性研究的文献,检索时限为建库至2021年12月。由2名研究人员独立筛选文献、提取数据、评价文献质量。采用Stata15.0软件进行Meta分析。结果:共检索到459篇文献,通过逐层筛选,最终纳入4篇文献,涉及24 953名受试者。Meta分析结果显示,血清铁蛋白含量与腰椎骨密度、股骨颈骨密度、股骨骨密度均无相关性[I2=85.9%,P=0.000,ES=-0.01,95%CI(-0.02,0.01); I2=42.7%,P=0.065,ES=-0.00,95%CI(-0.01,0.00); I2=47.5%,P=0.040,ES=-0.00,95%CI(-0.01,0.00)]。提取中高龄女性的数据进行亚组分析,结果显示,血清铁蛋白含量与腰椎骨密度、股骨颈骨密度、股骨骨密度均无相关性[I2=90.7%,P=0.000,ES=-0.02,95%CI(-0.05,0.01); I2=66.2%,P=0.031,ES=-0.01,95%CI(-0.02,0.00); I2=57.1%,P=0.072,ES=-0.01,95%CI(-0.02,0.01)]。基于血清铁蛋白含量和腰椎骨密度相关性的发表偏倚漏斗图呈对称分布,提示不存在发表偏倚。结论:现有证据表明,血清铁蛋白含量与骨密度无相关性。
Abstract:
Objective:To systematically review the correlations between serum ferritin(s-ferritin)level and bone mineral density(BMD).Methods:All articles about the correlations between s-ferritin level and BMD included from database establishing to December 2021 were retrieved from the China National Knowledge Internet,Wanfang Database,PubMed,Embase,Web of Science and Cochrane Library through computer.The articles were screened and the information was extracted independently by two researchers according to the inclusion and exclusion criteria.The methodological quality of research in the articles was evaluated by two researchers according to the quality assessment scale for cross-sectional study recommended by agency for health care research and quality(AHRQ)and a Meta-analysis was conducted by using Stata15.0 software.Results:Four hundred and fifty-nine articles were searched out.After screening,4 articles(24 953 subjects)were included in the final analysis.The results of Meta-analysis revealed that the s-ferritin level had no correlations with the BMDs of lumbar vertebrae,femur neck and femur(I2=85.9%,P=0.000,ES=-0.01,95%CI(-0.02,0.01); I2=42.7%,P=0.065,ES=-0.00,95%CI(-0.01,0.00); I2=47.5%,P=0.040,ES=-0.00,95%CI(-0.01,0.00)).The results of subgroup analysis on the data of middle-aged and elderly females showed that the s-ferritin level had no correlations with the BMDs of lumbar vertebrae,femur neck and femur(I2=90.7%,P=0.000,ES=-0.02,95%CI(-0.05,0.01); I2=66.2%,P=0.031,ES=-0.01,95%CI(-0.02,0.00); I2=57.1%,P=0.072,ES=-0.01,95%CI(-0.02,0.01)).Funnel plot was drawn based on the correlations between s-ferritin level and BMD of lumbar vertebrae,and the symmetrical funnel plot suggested that there was no publication bias.Conclusion:Available evidences suggest that there is no correlations between s-ferritin level and BMD.
