摘要:
目的:探讨高频超声在急性踝关节扭伤致分歧韧带损伤诊断中的应用价值。方法:采用美国GE公司的LOGIQ E9彩色多普勒超声诊断仪对85例临床疑似为急性踝关节扭伤致分歧韧带损伤患者的足踝部进行高频超声检查,详细观察分歧韧带的位置、走行及损伤情况,并根据其声像图特征总结分歧韧带损伤的类型。结果:85例患者中超声共检出分歧韧带损伤79例,其中跟骰韧带及跟舟韧带复合损伤57例、跟骰韧带单独损伤8例、跟舟韧带单独损伤14例(均跟骰韧带缺如); 分歧韧带正常6例,其中跟骰韧带缺如2例。按照《肌骨超声诊断》中韧带损伤的超声诊断分型,跟骰韧带损伤65例,其中Ⅰ型18例、Ⅱ型6例、Ⅲ型8例、Ⅳ型33例; 跟舟韧带损伤71例,其中Ⅰ型19例、Ⅱ型9例、Ⅲ型8例、Ⅳ型35例。19例Ⅳ型跟骰韧带及跟舟韧带复合损伤者因合并撕脱骨折块较大、移位明显及其他踝关节周围韧带严重损伤而进行手术治疗,术中探查发现分歧韧带损伤情况及撕脱骨折块位置、与母骨分离情况与超声检查结果完全吻合。结论:高频超声可清晰显示分歧韧带的位置、走行及损伤情况,并可对韧带损伤进行分型诊断,为临床后续治疗提供可靠的参考依据,可作为急性踝关节扭伤致分歧韧带损伤的筛查方法。
