摘要:
目的:探讨全膝关节置换术(total knee arthroplasty,TKA)前应用CT测量股骨后髁角(posterior condylar angle,PCA)的临床意义。方法:选取86例出生或久居苏浙沪地区的成年人,共涉及106膝,男47例53膝,女39例53膝; 年龄19~55岁,中位数38.5岁; 左膝34例,右膝32例,双膝20例。所有膝关节均无任何损伤、病变、骨骼畸形、骨质退变及内外翻畸形。垂直于股骨干解剖轴对股骨远端进行CT薄层扫描,选取经过股骨内、外上髁层面的CT图像,采用Auto CAD软件测量PCA。结果:本组受试者PCA1.65°~7.49°(4.80°±1.23°),男女PCA比较,差异无统计学意义(4.94°±1.24°,4.67°±1.21°,t=1.134,P=0.259); 左右膝PCA比较,差异无统计学意义(4.87°±1.24°,4.73°±1.22°,t=0.587,P=0.558)。使用矩法对样本行偏度系数和峰度系数计算,经双侧检验P值均大于0.05,符合正态分布。结论:PCA存在较大的个体差异,但不同性别及双侧PCA之间不存在差异。术前利用CT测定PCA,确定术中股骨后髁截骨的旋转角度可能有助于提高TKA手术的成功率。
