摘要:
目的:观察MAST Quadrant微创通道下伤椎置钉固定治疗胸腰椎骨折的临床疗效及安全性。方法:2009年12月至2011年6月,采用MAST Quadrant微创通道下伤椎置钉固定治疗胸腰椎骨折患者27例,男18例,女9例。年龄23～57岁,中位数31岁。所有患者均为单节段胸腰椎新鲜骨折,其中T11椎体骨折3例,T12椎体骨折7例,L1椎体骨折12例,L2椎体骨折5例。所有患者均无神经损伤表现,Frankle分级为E级。结果:手术时间60～95 min,中位数79 min。术中出血量50～150 mL,中位数86 mL。27例患者均获随访,随访时间6～12个月,中位数10个月。所有患者的骨折均获临床愈合,未发生切口感染及内固定失败等并发症。术后患者伤椎前后缘高度比[(0.86±0.05)%]、后凸畸形Cobb's角(5.63°±2.14°)、疼痛VAS评分[(2.61±1.27)分]及腰椎功能JOA评分[(22.65±1.93)分]均较术前[(0.35±0.09)%,(25.19°±8.68°),(7.22±1.05)分,(13.19±1.42)分]改善,且在末次随访时均维持较好[(0.82±0.03)%,(5.81°±2.22°),(1.26±0.53)分,(23.11±2.06)分]。结论:MAST Quadrant微创通道下伤椎置钉固定治疗胸腰椎骨折,手术时间短,术中出血量少,能有效缓解患者的腰背部疼痛症状、恢复并维持伤椎高度,且并发症少,是治疗胸腰椎骨折的有效方法,值得临床推广应用。
