摘要:
目的:系统评价数字健康技术(digital health technology,DHT)辅助的康复疗法治疗膝骨关节炎(knee osteoarthritis,KOA)的效果。方法:检索中国知网、万方数据库、维普网、中国生物医学文献服务系统、PubMed、Embase、Web of Science、Cochrane Library中比较DHT辅助的康复疗法和常规康复疗法治疗KOA临床疗效的随机对照试验,检索时限均为建库至2025年1月。由2名研究人员独立筛选文献、提取数据并评价纳入研究的偏倚风险后,采用R4.4.1软件的Meta程序包进行Meta分析。结果:共纳入17项研究,涉及1049例KOA患者,其中包括普通KOA患者和已行膝关节置换术(total knee arthroplasty,TKA)的KOA患者。Meta分析结果显示,与常规康复疗法相比,DHT辅助的康复疗法可显著降低患者的膝关节疼痛数字分级量表评分[TKA患者:SMD=-0.71,95%CI(-1.31,-0.12),P=0.018; 普通KOA患者:SMD=-0.60,95%CI(-0.77,-0.42),P=0.000]及西安大略和麦克马斯特大学骨关节炎指数评分[TKA患者:SMD=-0.55,95%CI(-1.08,-0.01),P=0.047; 普通KOA患者:SMD=-1.19,95%CI(-2.17,-0.21),P=0.017]; DHT辅助的康复疗法改善已行TKA的KOA患者站立行走时间的效果优于常规康复疗法[SMD=-0.60,95%CI(-1.17,-0.04),P=0.037],2种疗法改善普通KOA患者站立行走时间的差异无统计学意义[SMD=-0.10,95%CI(-0.39,0.20),P=0.513]; 2种疗法改善已行TKA的KOA患者膝关节活动度的差异无统计学意义[SMD=0.46,95%CI(-0.02,0.93),P=0.061],针对普通KOA患者的研究仅1项,未进行合并分析。Egger's检验结果提示不存在发表偏倚(P=0.078)。结论:现有证据表明,对于KOA患者,DHT辅助的康复疗法在减轻膝关节疼痛症状、改善膝关节功能方面的效果优于常规康复疗法。
Abstract:
Objective:To systematically review the therapeutic efficacy of digital health technology(DHT)-assisted rehabilitation therapy in treatment of knee osteoarthritis(KOA).Methods:All the randomized controlled trial(RCT)articles about the clinical outcomes of DHT-assisted rehabilitation therapy versus conventional rehabilitation therapy for treatment of KOA included from database's inception to January 2025 were retrieved from the China National Knowledge Infrastructure,Wanfang Database,Vip Database,Chinese Biomedical Literature Service System,PubMed,Embase,Web of Science and Cochrane Library.The pertinent articles were screened,the information was extracted and the risk of bias of the included researches in the articles was assessed independently by two researchers,and then a Meta-analysis was conducted using the Meta package in R software(version 4.4.1).Results:Seventeen articles(1049 KOA patients)were included in the final analysis,comprising both general KOA patients and post-total knee arthroplasty(TKA)KOA patients.The meta-analysis revealed that,compared to the conventional rehabilitation therapy,the DHT-assisted rehabilitation therapy significantly reduced patient's knee pain numerical rating scale(NRS)score and Western Ontario and McMaster Universities osteoarthritis index(WOMAC)score(TKA patients:SMD=-0.71,95%CI(-1.31,-0.12),P=0.018; general KOA patients:SMD=-0.60,95%CI(-0.77,-0.42),P=0.000; TKA patients:SMD=-0.55,95%CI(-1.08,-0.01),P=0.047; general KOA patients:SMD=-1.19,95%CI(-2.17,-0.21),P=0.017).Furthermore,the DHT-assisted rehabilitation therapy demonstrated superior efficacy in reducing timed up-and-go test duration compared to the conventional rehabilitation therapy in post-TKA KOA patients(SMD=-0.60,95%CI(-1.17,-0.04),P=0.037),while,the two therapies are comparable in improving this outcome in general KOA patients(SMD=-0.10,95%CI(-0.39,0.20),P=0.513).Moreover,the two therapies showed comparable therapeutic effect in improving knee range of motion in post-TKA KOA patients(SMD=0.46,95%CI(-0.02,0.93),P=0.061),while,only one study focused on general KOA patients,so no pooled analysis was conducted.The publication bias was analyzed based on the knee pain NRS score,and the result of Egger's test showed that there was no publication bias in the included articles(P=0.078).Conclusion:Available evidences suggest that the DHT-assisted rehabilitation therapy is superior to the conventional rehabilitation therapy in alleviating knee pain and improving knee function in KOA patients.
