摘要:
目的:观察经皮空心螺钉固定治疗骶髂关节复合体损伤的临床疗效及安全性。方法:2010年5月至2015年6月,采用经皮空心螺钉固定治疗骶髂关节复合体损伤患者18例,男11例、女7例。年龄23～51岁,中位数37.5岁。骶骨骨折11例,按照骶骨骨折的Denis分型,Ⅰ型9例、Ⅱ型2例; 骶髂关节骨折脱位7例。按照骨盆骨折的Tile分型,B型14例、C型4例。受伤至手术时间4～10 d,中位数7.5 d。随访观察骨折愈合及并发症发生情况,并于术后3个月采用Majeed骨盆骨折评分标准评定疗效。结果:手术失血量(40±10)mL。住院时间9～28 d,中位数16.5 d。所有患者均获随访,随访时间3～58个月,中位数34.5个月。骨折均愈合,愈合时间8～12周,中位数10周。术后3个月Majeed骨盆骨折评分(86.5±9.3)分,优11例、良6例、差1例。均未出现神经血管损伤及骨不连等并发症。结论:经皮空心螺钉固定治疗骶髂关节复合体损伤,创伤小、骨折愈合率高、疗效好、并发症少,值得临床推广应用。
