摘要:
目的:分析血清14-3-3η、趋化细胞因子CCL20(chemokine CCL20,CCL20)水平与膝骨关节炎(knee osteoarthritis,KOA)的关系。方法:选取2021年3月至2024年8月收治的KOA患者(KOA组)、无软骨损伤的膝关节周围韧带或半月板损伤患者(疾病对照组),以及同期在医院体检健康者(健康对照组)为研究对象。采用ELISA法检测受试者血清14-3-3η、CCL20水平。比较3组受试者及KOA组不同Kellgren-Lawrence分级受试者血清14-3-3η、CCL20水平的差异; 采用Spearman法分析KOA组血清14-3-3η、CCL20水平与Kellgren-Lawrence分级的相关性; 基于疾病对照组和KOA组受试者的数据,采用多因素Logistic回归模型分析血清14-3-3η、CCL20水平与KOA发病的关系,并采用受试者操作特征(receiver operating characteristic,ROC)曲线分析血清14-3-3η、CCL20水平对KOA的诊断价值。结果:①分组结果。KOA组最初纳入164例,失访和主动退出12例,最终纳入152例; 疾病对照组纳入166例; 健康对照组纳入154例。疾病对照组和KOA组的血清14-3-3η、CCL20水平均高于健康对照组(P=0.003,P=0.000; P=0.000,P=0.000); KOA组血清14-3-3η、CCL20水平均高于疾病对照组(P=0.000,P=0.000)。②血清14-3-3η、CCL20水平与Kellgren-Lawrence分级关系的分析结果。Kellgren-LawrenceⅢ、Ⅳ级KOA患者的血清14-3-3η、CCL20水平均高于Ⅱ级患者(P=0.000,P=0.000; P=0.000,P=0.000); Ⅳ级患者的血清14-3-3η、CCL20水平均高于Ⅲ级患者(P=0.011,P=0.011)。血清14-3-3η、CCL20水平均与Kellgren-Lawrence分级呈中度正相关(rs=0.511,P=0.000; rs=0.553,P=0.000)。③血清14-3-3η、CCL20水平与KOA发病关系的多因素Logistic回归分析结果。血清14-3-3η、CCL20水平升高均为KOA发病的独立危险因素[β=0.743,P=0.027,OR=2.103(1.089,4.063); β=0.886,P=0.002,OR=2.426(1.382,4.258)]。④血清14-3-3η、CCL20水平诊断KOA价值的ROC曲线分析结果。血清14-3-3η、CCL20水平单独及二者联合均对KOA具有一定诊断价值,对应曲线下面积分别为0.808、0.805、0.894,二者联合诊断的曲线下面积大于二者单独检测(Z=4.505,P=0.000; Z=4.730,P=0.000)。结论:血清14-3-3η、CCL20水平升高与KOA的发生、发展密切相关,二者不仅可作为评估KOA发病风险的生物学指标,其表达水平还能反映KOA患者的病情严重程度。
Abstract:
Objective:To investigate the relationship of serum 14-3-3η and chemokine CCL20(CCL20)levels with knee osteoarthritis(KOA).Methods:The patients with KOA(KOA group),and patients with ligament or meniscus injuries around the knee joint without cartilage damage(disease control group)recruited from March 2021 to August 2024,as well as the healthy individuals undergoing routine physical examinations during the same period at the hospital(healthy control group)were selected as the study subjects.The serum 14-3-3η and CCL20 levels in the subjects were detected by ELISA.The differences in serum 14-3-3η and CCL20 levels were compared among the three groups,as well as among subjects with different Kellgren-Lawrence(K-L)grades in the KOA group.The correlation of serum 14-3-3η and CCL20 levels with K-L grades in the KOA group was evaluated by Spearman correlation analysis.Based on data from the disease control group and the KOA group,the association of serum 14-3-3η and CCL20 levels with the onset of KOA was assessed by using multivariate logistic regression analysis,and the diagnostic value of these biomarkers for KOA was analyzed by using the receiver operating characteristic(ROC)curves.Results:①Grouping results.One hundred and sixty-six subjects were included in the disease control group,and 154 ones in the healthy control group.One hundred and sixty-four subjects were included in the KOA group initially.After excluding 12 cases due to losing to follow-up or withdrew voluntarily,152 ones were finally included in the KOA group.The serum 14-3-3η and CCL20 levels were higher in both disease control group and KOA group compared to healthy control group(P=0.003,P=0.000; P=0.000,P=0.000).Furthermore,the serum levels of both biomarkers were higher in KOA group compared to disease control group(P=0.000,P=0.000).②Analysis results of the relationship between the serum levels of 14-3-3η and CCL20 and K-L grades.KOA patients with K-L gradeⅢorⅣexhibited significantly higher serum 14-3-3η and CCL20 levels than those with gradeⅡ(P=0.000,P=0.000; P=0.000,P=0.000).Furthermore,the ones with gradeⅣhad significantly higher levels of both biomarkers than those with gradeⅢ(P=0.011,P=0.011).The serum 14-3-3η and CCL20 levels were both moderately positively correlated with K-L grade(rs=0.511,P=0.000; rs=0.553,P=0.000).③Results of multivariate logistic regression analysis on the relationship between serum 14-3-3η and CCL20 levels and the onset of KOA.Elevated serum 14-3-3η and CCL20 levels were both independent risk factors for the onset of KOA(β=0.743,P=0.027,OR=2.103(1.089,4.063); β=0.886,P=0.002,OR=2.426(1.382,4.258)).④Analysis results of ROC curves for the diagnostic value of serum 14-3-3η and CCL20 levels in KOA.The serum 14-3-3η level,serum CCL20 level,and their combination had certain diagnostic value for KOA,with the corresponding areas under the curve(AUC)of 0.808,0.805,and 0.894,respectively.The AUC of the combination in diagnostic performance was significantly greater than that of either biomarker alone(Z=4.505,P=0.000; Z=4.730,P=0.000).Conclusion:Elevated serum 14-3-3η and CCL20 levels are closely associated with the occurrence and development of KOA.The two biomarkers not only serve as biological indicators for assessing the risk of KOA onset,but their expression levels can also reflect the severity of the disease in KOA patients.
