摘要:
目的:探讨髌腱-股骨外侧髁摩擦综合征的MRI表现。方法:收集研究对象36例,髌腱-股骨外侧髁摩擦综合征患者11例(髌腱-股骨外侧髁摩擦综合征组),健康志愿者25例(健康对照组)。分析2组研究对象的膝关节MRI表现,在MRI图像上测量髌腱–股骨外侧滑车关节面(patellar ligament to lateral trochlear facet,PL-T)最短距离、胫骨结节–股骨滑车沟(tibial tubercle to trochlear groove,TT-TG)距离及Install-Salvati指数(Install-Salvati ratio,ISR),ISR>1.3判定为髌骨高位。对2组研究对象的测量结果进行比较。结果:髌腱-股骨外侧髁摩擦综合征组,膝关节FS-PDWI序列MRI上可见斑片状高信号影,提示髌下脂肪垫外上部水肿; 健康对照组均无髌下脂肪垫外上部水肿表现。髌腱-股骨外侧髁摩擦综合征组PL-T最短距离小于健康对照组,TT-TG距离、ISR均大于健康对照组[(4.27±1.23)mm,(8.01±1.83)mm,t=-6.579,P=0.000;(14.85±2.83)mm,(8.73±3.15)mm,t=5.879,P=0.000; 1.40±0.11,1.01±0.08,t=11.865,P=0.000]。髌腱-股骨外侧髁摩擦综合征组12个膝关节髌骨高位,健康对照组无髌骨高位。结论:髌腱-股骨外侧髁摩擦综合征MRI表现为髌下脂肪垫外上部水肿,PL-T最短距离减小,TT-TG距离及ISR增大。
