摘要:
目的:探讨人工肱骨头置换术治疗肱骨近端Neer四部分骨折的临床疗效和安全性。方法:采用人工肱骨头置换术治疗肱骨近端Neer四部分骨折患者21例,男8例,女13例; 年龄53～88岁,中位数73岁; 左侧7例,右侧14例; 均为新鲜闭合骨折; 合并肩关节脱位12例,合并肩峰骨折1例,合并桡骨远端骨折1例,合并腋神经损伤2例。受伤至手术时间2～10 d,中位数4 d。术后进行系统的肩关节功能锻炼。随访观察患者疼痛缓解、肩关节功能恢复及并发症发生情况。结果:本组21例患者均获随访,随访时间1～5年,中位数2.5年; 疼痛均缓解,肩关节功能均改善。患侧肩关节活动度,前曲70°～150°,中位数110°; 后伸18°～70°,中位数40°; 外展60°～130°,中位数81°; 外旋20°～40°,中位数22°; 内旋可达臀部至T10水平。采用Constant和Murley肩关节评分标准评价疗效,本组优4例、良11例、可5例、差1例。均无神经损伤、假体周围骨折、感染及假体松动、断柄和关节脱位等并发症发生。结论:人工肱骨头置换术治疗肱骨近端Neer四部分骨折,可有效缓解疼痛、改善肩关节功能,且并发症少; 但要求术者手术操作娴熟、精准,以确保术后肩关节功能的恢复。
