摘要:
目的:探讨骨盆轴向CT扫描在平向骶髂螺钉内固定术中的应用价值。方法:2011年9月至2012年8月,对39例正常骨盆轴向CT扫描图像进行系统分析,男31例,女8例。年龄18~66岁,中位数41岁。均排除腰椎骶化、骶椎腰化、骶髂关节病变及骨盆后环骨折后畸形愈合患者。分析骶骨翼斜坡与髂骨皮质密度影之间的关系,并测量骶髂螺钉钉道。结果:骶骨翼斜坡高于髂骨皮质密度影8例、平于髂骨皮质密度影20例、低于髂骨皮质密度影11例。平向骶髂螺钉钉道宽(12.7±3.9)mm; 一侧进钉点距离S1椎体中心矢状面(75.0±10.0)mm,距离对侧骶髂关节骶骨侧软骨(127.7±12.1)mm; 两侧进钉点之间相距(153.7±10.7)mm。结论:骨盆轴向CT扫描能够明确骶骨翼斜坡与髂骨皮质密度影之间的关系,有助于准确测量骶髂螺钉钉道,在平向骶髂螺钉内固定术中具有重要的临床应用价值。
