摘要:
目的:探讨后路椎间孔镜下开窗减压髓核摘除术治疗单节段神经根型颈椎病(cervical spondylotic radiculopathy,CSR)的临床疗效和安全性。方法:2014年1月至2015年5月收治15例CSR患者。男10例,女5例; 年龄27～51岁,中位数39岁; 均为单节段单侧CSR,C4～5 3例、C5～6 8例、C6～7 4例。所有患者均经6周以上非手术治疗,疗效欠佳。均采用后路椎间孔镜下开窗减压髓核摘除术治疗。测定患者术前及末次随访时的颈肩疼痛视觉模拟量表(visual analogue scale,VAS)评分、日本骨科学会(Japanese orthopaedic association,JOA)脊髓型颈椎病评分、简明健康状况调查表(short form 36 health survey questionnaire,SF-36)评分及椎体水平位移和椎体角度位移,并记录治疗及随访期间的并发症发生情况。结果:所有患者均顺利完成手术,术中未发生脊髓或神经根损伤。手术时间55～100 min,中位数78 min; 术中出血40～90 mL,中位数75 mL。14例患者获得随访,1人失访,随访时间9～26个月,中位数15个月。随访期间患者的神经根性症状均明显缓解,至末次随访时无1例复发; MRI提示原手术节段未见明显椎间盘突出,邻近椎体未见明显退变征象。与术前相比,末次随访时患者的颈肩疼痛VAS评分明显减小[(6.58±2.03)分,(1.23±0.75)分,t=9.250,P=0.000],JOA评分和SF-36评分均明显提高[(12.49±1.30)分,(16.32±0.69)分,t=9.737,P=0.000;(61.6±14.1)分,(79.1±16.6)分,t=3.006,P=0.006]; 椎体水平位移和椎体角度位移与术前相比,差异均无统计学意义[(1.12±0.63)mm,(1.16±0.38)mm,t=0.203,P=0.890;(4.98°±1.32°),(5.06°±1.15°),t=0.171,P=0.866]。结论:后路椎间孔镜下开窗减压髓核摘除术可有效减轻单节段CSR患者的颈肩疼痛症状,改善神经功能,提高患者的生活质量,而且不影响颈椎的稳定性,安全性高,值得临床推广应用。
