摘要:
目的:探讨基于CT周围神经重建图像判断骨和软组织肿瘤与邻近神经关系的价值。方法:选择36例行手术治疗的骨或软组织肿瘤患者进行研究。所有患者术前均行CT检查,并进行肿瘤邻近神经曲面重建。根据获取的CT周围神经重建图像判断骨和软组织肿瘤与邻近神经的关系,并以术中观察判断结果作为金标准计算诊断的灵敏度、特异度及约登指数。结果:33例完成肿瘤邻近神经CT图像曲面重建,并成功判断肿瘤与邻近神经的关系; 3例因肿瘤位于S3、S4水平,无法顺利进行周围神经图像重建。根据CT周围神经重建图像,10例邻近神经受侵、18例邻近神经受压、5例邻近神经未受侵或受压; 根据术中观察,8例邻近神经受侵、17例邻近神经受压、8例邻近神经未受侵或受压。以术中观察判断结果为金标准,基于CT周围神经重建图像判断邻近神经受侵的灵敏度为100%、特异度为92%、约登指数为0.92,判断邻近神经受压的灵敏度为94%、特异度为88%、约登指数为0.82。结论:基于CT周围神经重建图像判断骨和软组织肿瘤与邻近神经关系的准确性较高,具有较高的临床应用价值。
