经皮椎体后凸成形术治疗后壁破损的骨质疏松性椎体压缩骨折()

摘要:
目的:探讨经皮椎体后凸成形术治疗后壁破损的骨质疏松性椎体压缩骨折的可行性及临床疗效。方法:回顾性分析宁波六院脊柱外科采用经皮椎体后凸成形术治疗后壁破损的骨质疏松性椎体压缩骨折32例(37个椎体)患者的临床资料,其中女22例,男10例,年龄59~82岁,中位数69岁。致伤原因:跌伤18例,车祸伤6例,无明显外伤8例。手术节段:单椎体28例,两椎体3例,三椎体1例。术前CT示骨折线累及椎体后壁,部分骨块突入椎管。患者术前均无神经损伤症状。观察患者手术前后VAS评分改善、伤椎椎体前缘、中间的高度值恢复及并发症的发生情况。结果:32例患者均顺利完成手术,手术时间(42.5±16.2)min,骨水泥注入量(4.2±1.7)mL。骨水泥渗漏率为8.1%。末次随访时,伤椎椎体前缘的高度值由术前的(31.4±9.4)%恢复到(60.7±10.5)%、伤椎椎体中间的高度值由术前的(42.0±5.6)%恢复到(67.1±15.9)%、VAS评分由术前的(8.3±1.9)分降低到(2.9±2.2)分。结论:对于后壁破损的老年骨质疏松性椎体压缩骨折患者,只要术中操作得当,经皮椎体后凸成形术同样是一种安全有效的治疗手段。
