摘要:
目的:从病理学角度评价祛痰逐瘀法预防家兔激素性股骨头坏死的疗效。方法:按体质量均衡原则将30只健康SPF级新西兰 家兔随机分为4组:正常组(A组)6只,模型组(B组)8只,低剂量组(C组)8只,高剂量组(D组)8只。A、B组予生理盐水灌胃,10 mL·kg-1; C、D组分别以0.36 g·mL-1和0.72 g·mL-1的自拟祛痰逐瘀方混悬液灌胃,10 mL·kg-1。各组给药均每天1次,连续用药3周。第4周起在继续上述处理的同时,在B、C、D组家兔左侧大腿内侧肌肉 注射甲泼尼龙琥珀酸钠,20 mg·kg-1,每天1次,连用3周。药物干预结束后处死各组家兔取股骨制作切片,光学显微 镜下观察骨髓及骨小梁坏死情况,并应用Image-Pro Plus 6.0软件计算股骨头坏死率和坏死面积。结果:①各组股骨头坏死面积 比较,差异有统计学意义(F=21.104,P=0.000)。组间两两比较:C、D组与A组比较,差异无统计学意义(P=0.446,P=0.368); A、C、 D组坏死面积均小于B组(P=0.000,P=0.000,P=0.000); C、D组差异无统计学意义(P=0.901)。②股骨头坏死率。各组股骨头坏死 率比较,差异有统计学意义(F=18.029,P=0.000)。组间两两比较:C、D组与A组比较,差异无统计学意义(P=0.485,P=0.356); A、C 、D组坏死率均小于B组(P=0.000,P=0.000,P=0.000); C、D组差异无统计学意义(P=0.828)。结论:从病理学角度来看,应用祛痰 逐瘀法可有效预防家兔激素性股骨头坏死的发生。
Abstract:
Objective:To make a pathologic evaluation on the prophylactic effect of the method of expeling phlegm and removing blood stasis on the steroid-induced osteonecrosis of the femoral head(SONFH)in the rabbits.Methods:30 healthy New Zealand rabbits in SPF level were randomly divided into 4 groups according to body mass balance principle,6 cases in normal group(group A),8 cases in model group(group B),8 cases in low dose group(group C)and 8 cases in high dose group(group D).Rabbits in group A and group B were intragastric administrated with physiological saline(10 mL·kg-1 ),and rabbits in group C and group D were intragastric administrated with self-made suspension(10 mL·kg-1)which was made of expeling phlegm and removing blood stasis prescription with the dosage of 0.36 g·mL-1 and 0.72 g·mL-1 respectively. Administration of each group was carried out once a day for consecutive 3 weeks.From the beginning of the 4th week,20 mg·kg-1 methylprednisolone sodium succinate were injected into the internal side muscle of left upper legs of rabbits in group B,group C and group D,once a day for consecutive 3 weeks.After drug intervention,all the rabbits were sacrificed for making slices with their femurs.The necrosis of bone marrow and bone trabeculae were observed under light microscope,then the rates and areas of femoral head necrosis were measured through Image-Pro Plus 6.0 software.Results:①On the aspect of femoral head necrosis areas,there was statistical difference among the 4 groups(F=21.104,P=0.000); there was no statistical difference between group C and group A,between group D and group A(P=0.446,P=0.368); group A,group C and group D were all lower than group B(P=0.000,P=0.000,P=0.000); there was no statistical difference between group C and group D(P=0.901).②On the aspect of femoral head necrosis rates,there was statistical difference among the 4 groups(F=18.029,P=0.000); there was no statistical difference between group C and group A,between group D and group A(P=0.485,P=0.356); group A,group C and group D were all lower than group B (P=0.000,P=0.000,P=0.000); there was no statistical difference between group C and group D (P=0.828).Conclusion:The method of expeling phlegm and removing blood stasis is effective on prevention of steroid-induced osteonecrosis of the femoral head in the rabbits on the standpoint of pathology.
