摘要:
目的:探讨肱骨背外侧支撑钢板在肱骨髁部骨折中的应用。方法:15例肱骨髁间及髁上骨折患者,采用侧卧位肘后侧入路,暴露骨折部位,选择性克氏针临时辅助固定,肱骨背外侧支撑锁定型解剖钢板,固定肱骨外髁,恢复肱骨外侧柱,并连接内侧柱,术后石膏外固定3~5 d可行辅助被动关节功能锻炼,术后4周,主动功能锻炼活动,6~8周后逐步负重活动。结果:15例病人均得到随访,随访时间8~23个月,中位数11个月,无死亡,全部病例临床或骨性骨折愈合,未见肢体短缩、成角畸形和临近关节功能障碍发生等,无1例发生尺神经迟发性损伤。结论:支撑稳定固定是治疗肱骨髁部骨折的有效方法之一,合理的手术体位和暴露方法、稳定的支撑固定,早期有序的康复锻炼与有效内外固定相统一是治疗成功的重要环节,可以有效避免远期尺神经损害以及防止内置物失效的发生。
