摘要:
目的:观察改良经皮骨水泥椎间盘成形术(percutaneous cement discoplasty,PCD)治疗高龄腰椎滑脱症的临床疗效和安全性。方法:2018年4月至2021年4月采用改良PCD治疗高龄腰椎滑脱症患者13例。男4例,女9例。年龄70~86岁,中位数73岁。Meyerding分级,1度10例、2度3例。滑脱节段位于L3~L4 1例、L4~L5 4例、L5~S1 8例。记录骨水泥注入量、手术时间及手术前后的椎间隙高度。采用腰部疼痛视觉模拟量表(visual analogue scale,VAS)和Oswestry功能障碍指数(Oswestry disability index,ODI)问卷表评价临床疗效。观察术中及随访过程中并发症发生情况。结果:13例患者均顺利完成手术。骨水泥注入量(8.1±0.5)mL,手术时间(28±4)min。患者椎间隙高度,术前(6.13±0.91)mm、术后1 d(6.93±0.57)mm。所有患者均获 6个月随访。患者腰部疼痛VAS评分,术前(7.38±1.04)分、术后1 d(4.15±0.69)分、术后3个月(2.23±0.44)分; 患者ODI,术前(56.31±4.99)%、术后1 d(18.85±3.05)%、术后3个月(7.31±2.06)%。均未发生骨水泥渗漏、神经血管损伤、椎体骨折等并发症。结论:采用改良PCD治疗高龄腰椎滑脱症,能有效缓解腰部疼痛、改善腰椎功能,且安全性高。
