摘要:
目的:探讨3D打印技术辅助下切开复位跟骨锁定钢板内固定治疗劈裂型肱骨大结节骨折的临床疗效和安全性。方法:2016年4月至2020年6月,采用3D打印技术辅助下切开复位跟骨锁定钢板内固定治疗肱骨大结节骨折患者12例。男7例,女5例; 年龄29~77岁,中位数56岁; 左侧5例,右侧7例。骨折Mutch分型均为Ⅱ型,即劈裂型。合并肩关节脱位6例,合并肩关节脱位和臂丛神经损伤1例,合并肩袖损伤4例。受伤至手术时间3~6 d,中位数4 d。术前根据CT三维重建图像打印患侧肱骨近端骨折复位前和模拟复位后的3D模型,并参照模拟复位后的3D打印模型预弯、修剪跟骨锁定钢板用于骨折的固定。观察骨折愈合、肩关节功能恢复及并发症发生情况。结果:本组12例患者均获随访,随访时间12~26个月,中位数20个月。骨折均愈合,愈合时间8~13周,中位数10周。末次随访时Constant-Murley肩关节评分74~97分,中位数93分。评分最低的1例为合并肩关节脱位和臂丛神经损伤的患者。均无骨折再次移位、感染、肩峰下撞击征、内固定物松动等并发症发生。结论:3D打印技术辅助下切开复位跟骨锁定钢板内固定治疗劈裂型肱骨大结节骨折,骨折愈合好,有利于肩关节功能恢复,且并发症少。
